Teens
New York State and federal laws mandate confidentiality for certain health issues. To ensure privacy, teens between the ages of 12-17 years will have access to their health records through their own MyCareCorner account and/or may authorize parental/guardian access. If you would like your parent or guardian to create an account on your behalf, or have access to your health record, you will be required to sign the MyCareCorner Access Request Form. You may revoke access at any time.
